Building on traditions - A vision for the future

Swing Beds

What is the Swing Bed Program?

Swing Bed is an alternative service when a patient no longer needs acute care, yet they may not feel strong enough to return home.

Our Swing Bed post-acute rehabilitation service is designed to provide patients with individualized, in-hospital care and physical rehabilitation to help them reach an optimal level of functioning. This post-acute care is designed for patients who are discharging from acute care, but need temporary additional care that cannot be provided at home or in a long term care facility, and could include patients recovering from an illness, injury or surgery.

Through a combination of first rate rehabilitative therapies, attentive nursing care and medical supervision, patients gain the strength, functionality, balance and range of motion they need to care for themselves with confidence.

What Patients are Qualified?

Qualified patients who have been admitted to Hills & Dales General Hospital for acute care can transfer to our Swing Bed Program for their rehabilitation phase of recovery and other skilled services without ever leaving the hospital.

Patients who are in another acute care facility can transfer to our Swing Bed Program, allowing you to be close to home, family and friends during your recovery.

You’ll find that Medicare and some other select insurance companies cover post-acute rehab services. Our admissions coordinator will assist with determining whether your insurance covers post-acute services.

If you think the Swing Bed Program might be right for you, please contact our Admissions Coordinator at 989-912-6433.  If you are at another hospital or facility, you can discuss the Swing Bed Program with your discharge planner.

Testimonials

I actually found that I really enjoyed my time in the Swing Bed Program.  I had a hip replacement and had no intention in staying longer than a few days.  The hospital was impeccably clean, the staff was great, the activities were fun, and I healed very quickly.  It was really nice to have surgery and do my recovery without ever leaving the hospital.

The entire staff was excellent! Everyone was courteous and professional at all times. I will always recommend (Hills & Dales) for any rehab. The therapists were able to get me up to speed quickly and painless.

The Hills & Dales staff was exceptional throughout my PT. Scheduling early/late appointments is helpful for me and is not an option that I could find anywhere else. My therapist took the time to listen to my concerns and always worked to adapt my therapy to assist with them. Thank you!
